You can make a log file of the console to find out what error it is getting. Before you load your map, bring up the console and type:

logfile 2

Then after it crashes, find qconsole.log in mohaa/main and read it to find the error.

options

You can turn off that auto game start up in Mbuilder. Just hit the "Other options" button to see the menu.

Have it place the bsp somewhere that you can find it. Then move the bsp into the main/maps/dm folder. In the game, go to multiplayer and find your map in the list of Team Deathmatch maps. But don't forget to turn on that logfile 2 option in the console.
