First off, make your door. Give it these key/values-
targetname door1
spawnflags 128 (This makes your door only triggerable from wherever you're going to place your trigger_use. More on that in a moment)

Construct a switch, or use a static model, and place it where you want. Now, right-click, and select --> trigger_use. Resize the trigger so that it fits your switch. Give it these key/values-

target door1

You should now see a red arrow connecting your trigger_use to the door.

That's it, you're done!

yes, use the key "wait". I believe default value is 3, and never close is -1.
I think.

good luck.

THe door also has a check box named "Toggle" which will causes the door to wait in both the start and end states for a trigger event.

If "Targeted" is set, the builtin trigger will be disabled and any trigger that targets the door by its targetname will ativeate it.

All of this stuff can be seen in the gray box of the entity window.
