1) I was wondering, that how can i make a bunker inside a mountain, which is created with "lod terrain, create from brush".
2) How do i make ladder? lol

There is a facet editor for LOD terrain. With it you can make holes in the terrain for your bunker. The MOH Terrain Editing Manual in the docs folder tells all about it--part IV.

Higlight the little triangles (facets) you want to delete and put a checkmark in the delete box. Then hit done.
To see the deleted triangles, you must click on LOD Terrain > Color Surface Flags.
They are not really deleted because you can still undelete them, but they will not be seen in the game.

how do u select individual triangles? all i can do is select patches that are 512 x 512

You select them the same way you would select a single face on a brush (Ctrl + Shift + LMousebutton),
but the facet editor must be showing.
Btw, Your terrain should be textured before you do it.
You can undelete triangles by highlighting them, removing the delete checkmark, and hitting done.
The deleted triangles will look sort of grayish-yellow in the editor--with LOD Terrain > Color Surface Flags turned on.

while we're on LOD terrain, how can i make it so that it isn't just a white patch of ground? so the texture appears i mean....

my LOD looks like a big white block really. i can't tell the texture. so i'm always lost on wtf the texturing looks like and where the textures are.

Press shift+F, select the triangles u want to delete, then select in the edit facet flags pop-up(shift+F): delete, then press done.
You will still see the triangles in radiant, but not in the game
