STEP ONE............ make a ladder. Use caulk to make it. Then select a wood texture to put on it

STEP TWO.......... find the ladder brush. Make it 1 foot taller than the ladder you made.

Make it AS wide as the ladder

And Make it about 4 inches thick

STEP THREE.......... With the ladder brush highlighted......... in the 2d view....... right click.....goto func/ladder

STEP FOUR....... after doing the above......... and while the ladder brush is still highlighted............. hit the "N" key.

In the bottom of the entity inspector is two small boxes.

The top one says ............. key......... in that one type angle
the bottom one says .........value....... in that one type 90

hit enter.......... deselect ladder brush so it's no longer highlighted.
save and recompile map.............. if it doesn't work........ change the angle.............. 0/180/270/360..........ETC.........

Also make sure the common ladder brush is higher than the actual ladder or you wont be able to get off the top of it

You have something in the players way. Probably a ceiling. You are going to have to remove it, or make it higher so the player can get off at the top.

Yep, you need room behind and above the player.
