but whatever i do, the scale always seems to be wrong, either the map is made for dwarfs or for giants, but never as it should
so, my question is how many units equal 1 meter?

Doors for me are 112x56 (stock they are 128x64...too big)
Room height for me is around 160/172.
Windows as squares i'd have 64x64 but they can be anything that suits the building.
Ladders...well um as high as you need it to be m8.
Ohh and while I remember..try to keep your dimesions to denominations of 8 or 16 or for that matter any denomination of the number 2 ie: 2/4/8/16/32 - This keeps the map clean and much simpler to fit texures.
If you don't understand i'll elaborate but avoid units with odd number sizes for ex: 15x135
