Minimum map requirements: need to know

Hi all... I have combed the forums with little success on finding a solution to my problem.

I am creating a map. I have all of my folder structure set up and all of my scripts are correct. THe reason I know this is that I took a .bsp file from another .pk3 file, renamed it to the name of my map, zipped it and ran it, and it works fine.
My map file has no leaks in it and I have 2 spawns... 1 axis... 1 allied. I have my lights set up too.
Once the map is it built, I stuff it inside the maps/dm folder inside the pk3. I can load it as a death match map. But once the load screen is done, it kicks me back to the fan room of MOHAAS. Could someone point me in the right direction so I know at least what minimum settings need to be in place before I compile my map? I am pulling my hair out on this one. Note: I am able to get in the map if I load it in the command line. It fails when the map is zipped in a pk3.

before you load the map, open console type logifle 1 and oad your map... it crashes, exit moh, goto main and send us the qconsole.txt ;)

Do you have a different version of the map as a BSP file in your maps/dm directory in addition to the .pk3 version?

No a diff version would crash immediately.

enable console, when the game kicks you out open console and look at any errors.

Guys,
Thanks for the awesome input.
I've narrowed it down to the spawns. When I have an info_player_axis and an info_player_allied spawn point, it dies after the loading screen and goes back to the fan room. When I put a info_player_deathmatch in, I can enter the game in spectate, but as soon as I join a team, it kicks me out back into the fan room. What spawn setup should I use for a deathmatch map? you should have one of every spawn in an MP map.

remember to put in info>player>start as well.
