Sometimes the View/filters turn themselves on. If any are on, their names show up in the status line at the very top of the editor.
Also, if there is a bad line of text in the .map file, it will cause some entities to be hidden and a small untextured box will show up near the center of the grid. You can open the .map file with a text editor, like Wordpad, to see if your entities are still there and try to find the bad line.

There can be several reasons:
You said that the whole brush had disappeared, but it problaby were just the textures
1. You rotated the textures too much with Free Rotation
2. You copied too much
3. You edged the brushes wrongfully...

OR if you save your map then come back and open it, with hitting File>Open>yourmap then it might make things disappear and textures look weird. So instead hit File>Load>yourmap and it loads it right.

If Use quake3 .map format is check in preferences and a surface parm is set it can cause a problem like this also your detail brushes won't stay detail . Also sometimes you can get a bad brush or entity , use edit->mapinfo and look how many brushes it says you have in your map . Then open your map in a text editor and look for that numbered brush -1 , brushes start at 0 , this may be a bad brush that is keeping all the brushes after it to not load . Any way if the number of brushes in the text editor is greater than the number reported by map info then you know it is probably a bad brush . Entities are put in the .map file after brushes so if this is the cause then every thing after the error won't load includeing entities .

Check your main folder and see if you moved the entdef.pk3
When the info> entity disappeared on my mohradiant it was because i moved that pk3 outta my main because i had a twl match.....
Same things are missing when u right click as were missing when i right clicked.
