As for the door the you need to make sure the angle of the door is set correctly. Press 'N' and click on the 90 button if a door is like this --- in the top view, and if it is like this | you can leave it as it is or press the 360 button. The alwaysaway key is set to 1 as default so you only need to set this to 0 if you really want a door that just opens one way. This has been answered thousands of times. try a search to find solutions the most common problems.

As for the other questions I wouldn't know where to start. Either you make the handrail out of smaller brushes. You do know that you can change the grid size, right? if not, press 1 to 7 for different sizes. 1 being the smallest. Or you could do it with a masked texture such as 'algiers/railing_natestah' or 'central_europe/balcny_rail_set1' just make sure you only texture one face with the masked texture and the other with 'nodraw'

welcome, choncey!

here is a picture to help you see what rOger is talking about.

Image

use the xy top view to tell the editor what angle to use for the door as shown in the picture. all of the hinges in the example are in the center. look at the 3d view to see what i am talking about(red brush inside of the door)

hope this doesn't confuse you more.
