Mp3's work fine, theres some sound parms which control volume, pitch and a few other things.
Heres an example:
aliascache totalwar sound/nazaire/totalwar.mp3 soundparms 1.0 0.0 0.0 0.0 500 3000 item loaded maps "dm obj"
// soundparms parameters are: <BaseVolume> <RandVolume> <BasePitch> <randPitch> <minDist> <maxDist> <Channel> <Loaded/Streamed> <Maps>
So the 4th set of numbers control the pitch ( speed ) of the mp3. Set it to 0.0 and it shouldnt deviate from the original pitch.

do mp3's have to be streamed as well from outside a pak ?

For example the path to this mp3 I have playing in my map is this:
sound/nazaire/totalwar.mp3
so just replicate this folder structure but adapted to your file names and include the folders and sound file in the pk3, the sound should play from your pk3, wont need to be outside the pk3.

I've often wondered about that, but I have had custom mp3's in 3 of my maps and no-one has complained about not hearing it. I always put them in the sound folder in my custom pk3. Could be the new workaround has solved any problems there were.
Its great having sound at last, sound is so important in imersing a player into your level.
